Position Purpose
The Clinical/Staff Pharmacist performs all the essential functions of an on-site consultant pharmacist and staff functions of a pharmacist in a closed-door long-term care pharmacy. The day to day functions of this position with be delegated to the Clinical/Staff Pharmacist by the Pharmacist-in-Charge based on operational needs.
Essential Position Functions
- Provide on-site clinical pharmacist functions at the direction of the client(s) such as, but not limited to:
- Participate in client interdisciplinary care team (IDT) meetings
- Provide medication chart reviews and make appropriate clinical recommendations
- Provide medication regimen intake reviews during the onboarding of new patients to the client
- Provide medication assessments during transitions of care
- Assist in fall risk assessments
- Provide polypharmacy medication evaluations
- Provide medication therapy management (MTM) including comprehensive medication (CMR) and targeted medication review (TMR)
- Assist in chronic disease state management for medication dependent diseases at the discretion of the client(s)
- Other clinical functions as assigned
- Perform all duties of a staff pharmacist including but not limited to:
- Review prescriptions for data entry accuracy
- Communicate with providers to receive and clarify prescriptions
- Transfer prescriptions to and from other pharmacies
- Review medications profiles to identify drug therapy opportunities
- Check final product for accuracy before dispensing
- Act in supervisory role in the absence of the Pharmacist-in-Charge
- Provide pharmacist consultation to patients as needed
- Provide evening, weekend, and holiday coverage during scheduled on-call rotation and as required
- Adheres to PersonifilRx’s quality and customer service standards, policies and procedures, state and federal guidelines
- Attend company meetings as requested
- Other duties as assigned by Pharmacist-in-Charge
Minimum Requirements of the Position
- BS degree in Pharmacy or Pharm.D
- Three to five years of experience as a clinical or long-term care pharmacist is preferred
- Working knowledge of Microsoft Word, Excel, PowerPoint, and Outlook
- Must have a valid Pharmacist License in Massachusetts and Rhode Island
